Introduction

Information covered by this scheme is only about the primary, general or personal medical services we provide under contract to the National Health Service.

How much does it cost?

There will not usually be a charge for supplying information. However, if the request involves photocopying, it may be necessary to make a charge. In all cases, we will let you know if there is a cost before we start work so you can choose whether to pay it or to modify your request.

How the information is made available

All requests must be made in writing and should state name, contact address and phone number. All seeking detail should be precise in details. All the information within each class is available in hard copy from:

Class of information

All information at the Mitcham Family Practice is held, retained and destroyed in accordance with NHS guideline. Our commitment to publish information excludes any information which can be legitimately withheld under the exemptions set out in the NHS Openness Code or Freedom of Information Act 2000. Where individual Classes are subject to exemptions, the main reasons are e.g the protection of commercial interests and personal information under Data Protection Act 1998. this applies to all class within the Publication Scheme. The information on this Scheme is grouped into 7 classes:

Class 1. Who we are and what we do

This practice has a local Personal Medical Services contract. The contract is with NHS England , Primary Care Commissioning, South LLondon Area Team, Southside, 105 Victoria Street, London, SW1E 6QT

The practice aims to follow National Institute for Clinical Excellence (NICE) and national Service Framework Guidelines. Copy of these can also be found on the Nice website (www.doh.gov.uk)

A full list of local General practice can be also found there too. The Mitcham Family Practice provides general medical services for the geographical area of Mitcham.

We are friendly team of professionals whose aim is it to look after you and the health needs of your family. All our staffs are trained to put your needs first.

Practice nurse

She is available from 09.00 until 18:00 Monday, Tuesday and Wednesday 9am to 4pm. The nurse sees patients for a variety of procedure and advice. A full list of the service we provide is detailed in Class 7.

Admin/reception staff

We have four part time admin/ receptionists, who are all trained and some are very experienced. Anything you tell them will be treated in absolute confidence.

Practice Manager

We have one full time Practice manager. She is there to help you all your concern./ complaints.

Some information will be withheld, including personal, confidential information about individuals which is protected by the Data Protection Act 1998.

Class 2. What we spend and how we spend

The Mitcham Family Practice receives reimbursement from NHS London according to its contract (as per Class 1 above) in exchange for services provided for patients.

There may be circumstances where material the practice cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes under the Act has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

Class 3. What our priorities are and how we are doing

Practice priorities are:

  • To achieve all local and national Target
  • To carry out review with the NHS London and Patient Representative of yearly patient survey results and institute the agreed action plan.

Class 4. How you make decisions

The practice carry out regular meeting with the whole team and attached ancillary staff (District Nurse, Health Visitor, Midwifes, and Mental Health Team, Community Diabetic Nurse) in order to discuss various issues and making the appropriate decision. Minutes of meeting are held on record (up to the last 2 years) for every meeting.

Out of hour arrangement

The surgery is cover by the out of hour provider Harmoni, when closed in the evening, weekend, and bank holiday. Patient can access the out of hour provider by ringing to the surgery or ring direct 111.
